The grand prize will be awarded to one lucky winner. This weekend getaway is designed to let a primary caregiver have a weekend away as a sort of respite. Or as we say, a chance to “Unwind This Spring.”

Nomination Prize
You may not be a full time caregiver for anyone but that doesn’t mean you can’t win something awesome! If you fill out our entry form and nominate someone deserving for this award and they win, you will automatically receive a $100 University Mall Gift Card (if University Mall isn’t convenient for you, we can get you a similar prize of the same dollar value).
How to Enter
There are two different ways to enter. You can either fill out our quick form found at the bottom of this page or you can enter the “Unwind This Spring” giveaway on our Facebook page. We’d love to have you enter and “Like” our Facebook page as we’ll constantly be updating that page with useful tips on home health care and hospice services can benefit you or people you care about. At the same time we understand that not everyone has a Facebook account so we’ve provided the secondary option of filling out the form on this page.
