OFFICIAL RULES FOR A-Plus Home Care and Hospice’s “UNWIND THIS SPRING” SWEEPSTAKES
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS SWEEPSTAKES. MAKING A PURCHASE DOES NOT IMPROVEYOUR CHANCES TO WIN. Sweepstakes runs on http://www.facebook.com/aplushhh and http://aplushhh.com from 5:00 PM Mountain Time (“MST”) on 4/13/11 to 11:59 AM MST on 5/13/11 (“Promotion Period”).
Eligibility: Open to legal residents of the U.S., 18 years of age and older residing in Utah. Employees of A-Plus Home Care and Hospice and and their immediate family members (parent, child, sibling, and spouse) and persons living in the same households of each are not eligible. Void where prohibited by law.
How to enter: Go to http://facebook.com/aplushhh or http://aplushhh.com during the Promotion Period, click on the “Sweepstakes” link on the Facebook page or “Enter Sweepstakes” link at the top of the microsite page and follow the entry instructions. Fully complete the required information, including your first/last name, state, email address and birth date. You will also be given the opportunity to Invite Friends to enter the Sweepstakes. A-Plus Home Care and Hospice may email you company updates and announcements, but you can unsubscribe at any time. Limit one entry per email address. Any attempt to obtain more than one entry per email address by using different or multiple identities, log-ins, any automated entry system, or any other method is prohibited and will result in disqualification.
We have also created the ability to nominate someone for the prize package. When filling out the entry form simply enter the name of the person you’d like to nominate and if your name is drawn, we will follow up with you on how to get the award to the person you nominated.
Prizes:
Grand Prize: A 2 Night Stay at the Homestead Resort in Midway, Utah including 2 – 18 hole rounds of golf (not including cart) and dinner for 2 at the Zermatt Resort Restaurant (also located in Midway, UT). The prize package also includes 10 hours of quality care from a State Licensed Certified Nursing Assistant. Any prize not claimed or returned as undeliverable will be awarded to an alternate entrant. No substitution or transfer of prizes permitted. Any expenses relating to the acceptance and use of a prize not specified are the sole responsibility of the winners. Sponsor makes no representation that a prize can be sold or traded at the stated value. Taxes or other assessments on all prizes are the sole responsibility of the winners.
Runner-Up Prize: A $100 Gift Certificate to University Mall in Orem, UT. If the winner of the grand prize wins because someone nominated them, the entrant who nominated them will automatically win this prize. If the winner of the grand prize won on their own, the runner-up prize will be awarded by random drawing.
Sweepstakes Drawing Dates: Random drawings will be conducted by Webster Interactive, Inc. (“Administrator”) to award the Grand Prize and the runner-up prize on May 13, 2011 from among all entries received. The odds of winning depend upon the total number of eligible entries received during the Promotion Period.
Privacy: Your participation in this Sweepstakes constitutes your consent for Sponsor and Administrator to use your personal information only for the purpose of administering the Sweepstakes and awarding the prizes, unless you have opted to receive e-mail communications from Sponsor.
General: Federal, state, and local taxes and any expenses relating to the acceptance and use of a prize not specified are the responsibility of the winners. All entries are subject to verification prior to awarding a prize. By entering, participants release and hold harmless Sponsor, Facebook, and Administrator and their parents, subsidiaries, affiliates, directors, officers, employees and agents from all and any injury, damage, loss, expense, accident, delay, inconvenience, or irregularity arising out of or relating to any prize won in this Sweepstakes, even if caused by the negligence of Sponsor. Sponsor is not responsible for printing or typographical errors in any Sweepstakes-related materials; for lost, stolen, late, misdirected, damaged, incomplete, illegible, or postage-due mail, or for transactions that are lost or fail to enter the processing system, are processed, reported, or transmitted late, or are lost for any reason including computer, telephone,
paper, or any other malfunction including human error. Sponsor, at its sole discretion, reserves the right to cancel or modify this Sweepstakes, in whole or in part, at anytime without notice if fraud or technical failures (including but not limited to infection by computer virus, bugs, tampering, unauthorized intervention, or any other causes beyond the control of Sponsor and Administrator) or any other reasons whatsoever interfere with its conduct as contemplated by these Official Rules. If the Sweepstakes is cancelled for any reason before completion of the Promotion Period, all entries received up to the point of termination will be included in a random drawing to award the Prizes. Submitted materials become the sole property of Sponsor and will not be returned. By entering, you accept and agree to these rules and the decisions of Sponsor and the Administrator which shall be final and binding in all respects relating to this Sweepstakes.
ANY ATTEMPT TO DELIBERATELY DAMAGE THE ENTRY SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, A-PLUS HOME CARE AND HOSPICE RESERVES THE RIGHT TO SEEK REMEDIES ANDDAMAGES (INCLUDING ATTORNEYS’ FEES) TO THE FULLEST EXTENT OF THE LAW.
Force Majeure: The failure of Sponsor and Administrator to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, terrorism, act of public enemies, actions of governmental authorities — outside of the control of the Sponsor and Administrator (excepting compliance with applicable codes and regulations) — or other force majeure event will not be considered a breach of these Official Rules.
This promotion is in no way sponsored, endorsed, administered by or associated with Facebook. By participating in this Sweepstakes, you agree that you are providing your information to A-Plus Home Care and Hospice and their Sweepstakes Administrator and not to Facebook.
